Eligibility

Section 970.220  Eligibility Eligible applicants include: a)         Physicians licensed in Illinois to practice medicine in all of its branches, licensed hospitals in Illinois, certified laboratories in Illinois, certified mammography professionals and facilities, post-secondary higher education institutions in Illinois, and other medically affiliated organizations in Illinois and persons who are Illinois residents or sponsored by an Illinois facility guaranteeing benefits to Illinois residents. b)         All certified local health departments that provide public health programs as defined in Section 615.200 of the Local Health Protection Grant Rules (77 Ill. Adm. Code 615.200).
