Section 230.560 Coordination
of Services
In administering services under
Title III-D, each area agency on aging shall establish and implement a
documentable method to coordinate with other community agencies and voluntary
organizations providing:
a) counseling and training for family caregivers and support
service personnel in management of care;
b) functional and needs assessment services;
c) assistance with locating, arranging for, and coordinating
services;
d) case management; and
e) counseling prior to admission to nursing home to prevent
premature institutionalization.
