Home Visit

Section 240 Section 240.620  Home Visit a)         Determinations and redeterminations of need for CCP services shall be administered during a visit to the home of the participant, except when conducted in the prescreening process or when a CCP participant has been hospitalized or placed in any type of institution and will be discharged to the community in less than 60 calendar days. b)         A home visit shall be conducted, and a redetermination of need administered in the participant's home, within 15 calendar days after a participant's discharge from a hospital or other institution. c)         A home visit shall not be required in the conduct of determinations or redeterminations of need following hospitalization for routine procedures, as defined in Section 240.160.
