Replacement of Diesel Emission Inspection Report

Section 460 Section 460.620  Replacement of Diesel Emission Inspection Report a)         A replacement Diesel Emission Inspection Report may be obtained by submitting a written request for a replacement card along with a check or money order in the amount of $5 made payable to:  Treasurer, State of Illinois.  No cash will be accepted. b)         The written request must contain general information about the vehicle (i.e., VIN, make, model and year), as well as a contact person's name, address, and telephone number.  The replacement Diesel Emission Inspection Report will expire on the same date the original report was scheduled to expire. c)         All replacement requests shall be submitted to: Illinois Department of Transportation Bureau of Investigation and Compliance 2300 South Dirksen Parkway Springfield, Illinois  62764 DOT.VehicleInspectionUnit@illinois.gov
