415 ILCS 45/16

(415 ILCS 45/16) (from Ch. 111 1/2, par. 516) Sec. 16. Applications for certification shall: (a) Be on forms prescribed and furnished by the Agency; (b) Contain statements showing applicant's education and experience; (c) Contain not less than 3 references; and (d) Be accompanied by the proper fee as provided in Section 22 of this Act.
