405 IAC 5-18-3 Inpatient and outpatient laboratory facilities; limitations

Sec. 3. (a) To be eligible for reimbursement, a laboratory service must be ordered in writing by a physician or other practitioner authorized to do so under state law. (b) Laboratories performing the services must bill Medicaid directly unless otherwise approved by the Centers for Medicare and Medicaid Services.
