410 IAC 18-4-2 Documentation required to obtain death certificate

Sec. 2. The applicant of a death certificate shall provide the following to the state registrar upon request: (1) Valid identification. (2) Other documentation as requested by the state registrar. (3) A check or money order payable to the Indiana department of health for the correct amount required by law.
