410 IAC 3.2-5-2 Family responsibility to disclose information

Sec. 2. A person who is applying for CSHCN services or a family of a child enrolled in the CSHCN program shall immediately do the
following:
(1) Inform the child's CSHCN care coordinator of changes in their financial circumstances that may affect the family's financial
eligibility.
(2) Inform the child's CSHCN care coordinator of any plans that the family has to move to a new address within the current county
of residence, to another county, or to another state.
(3) Inform the child's CSHCN care coordinator of changes in the child's medical condition that may affect the child's medical
eligibility.
(4) Inform the child's CSHCN care coordinator of any changes in the child's health insurance benefits.
