460 IAC 14.1-4-6 Amendments to the record of services

Sec. 6. An applicant or a participant may submit a written request to their vocational rehabilitation counselor to amend information in their record of services that they believe is inaccurate. The VR program shall correct any inaccurate information. If information is not corrected, documentation of the request will be included in the record of services.
