460 IAC 6-21-2 Documentation required

Sec. 2. A provider of environmental modification supports shall maintain the following documentation regarding support provided to an individual: (1) The installation date of any adaptive aid or device, assistive technology, or other equipment. (2) The maintenance date of any adaptive aid or device, assistive technology, or other equipment. (3) A change made to any adaptive aid or device, assistive technology, or other equipment, including any: (A) alteration; (B) correction; or (C) replacement.
