460 IAC 6-25-1 Provider of health care coordination services

Sec. 1. Coordination of the health care for an individual shall be the responsibility of either of the following: (1) A provider of health care coordination services. (2) The provider identified in an individual's ISP as responsible for the health care of the individual.
