460 IAC 6-32-2 Documentation required

Sec. 2. A provider of specialized medical equipment and supplies supports shall maintain the following documentation regarding support provided to an individual: (1) The installation date of any adaptive aid or device, assistive technology, or other equipment. (2) The maintenance date of any adaptive aid or device, assistive technology, or other equipment. (3) Any change made to any adaptive aid or device, assistive technology, or other equipment, including an alteration, correction, or replacement.
