460 IAC 6-9-2 Adoption of policies and procedures to protect individuals

Sec. 2. (a) A provider shall adopt written policies and procedures regarding the requirements of sections 3 and 4 of this rule. (b) A provider shall require the provider's employees or agents to be familiar with and comply with the policies and procedures required by subsection (a). (c) Beginning on the date services for an individual commence and at least one (1) time a year thereafter, a provider shall inform: (1) the individual, in writing and in the individual's usual mode of communication; (2) the individual's parent, if the individual is less than eighteen (18) years of age, or if the individual's parent is the individual's legal representative; and (3) the individual's legal representative if applicable; of the policies and procedures adopted pursuant to this section.
