511 IAC 14-1-2 Application requirements for all licenses issued by the department

Sec. 2. An initial or renewal application for any license must include the following: (1) A completed electronic application approved by the department. (2) The fee prescribed by the department, which will be submitted electronically. (3) All necessary evidence of eligibility, as requested by the department.
