655 IAC 2-4-2 Application for certification based on reciprocity

Sec. 2. (a) An individual applying for a certification based on reciprocity must file an application on a form prescribed by the department. (b) An application must be filed for each certification based on reciprocity. (c) An application for a certification based on reciprocity must include the following: (1) Identifying information about the applicant. (2) Evidence the applicant has completed the same or substantially similar training and testing for the certification as that required by the education board, including: (A) a certification at the same level by the International Fire Service Accreditation Congress, National Board on Fire Service Professional Qualifications, or United States Department of Homeland Security; or (B) proof of successful completion of a certification program based on the same certification standards as the certification approved by the education board. (3) A sworn statement by the applicant affirming the information provided is true and accurate. (4) A nonrefundable processing fee of fifty dollars ($50) under IC 22-14-2-7(d).
