760 IAC 1-68-17 Fees

Sec. 17. The following fees apply to MEWAs:
(1) An applicant shall pay a nonrefundable fee of three hundred fifty dollars ($350) for filing an application for a certificate of
registration.
(2) Each MEWA holding a certificate of registration shall pay an annual internal audit fee of one hundred dollars
($100).
(3) A fee of fifty dollars ($50) shall accompany the filing of the annual report required by section 9 of this rule.
(4) A fee of thirty-five dollars ($35) shall accompany each form filed as required by section 12 of this rule.
