824 IAC 2-4-5 Amended licenses

Sec. 5. (a) A licensee shall complete a license application form when any of the following occur: (1) Adding an additional facility. (2) Change of business name. (3) Change in owners. (4) Change in type of license. The form shall be provided by the agency and shall contain the same information as the initial application, but it shall have a place for the applicant to indicate it is an amendment. (b) After receipt of the amended application, a determination shall be made as to whether, under the Act, any of the following should occur: (1) An increase or decrease in the bond. (2) An increase or decrease in the insurance. (3) An additional license fee. (4) A new license issued and the previous license recalled. (c) If an additional facility is added to the license, the appropriate facility fee shall apply. (d) If there is only a change in the name of the business or the organization type and the stockholders or owners remain the same, there shall be no additional charge until the next renewal period.
