845 IAC 1-4.1-6 Address; change of name

Sec. 6. (a) Each licensee shall inform the board, in writing, of all changes in address or name within fifteen (15) days of the
change.
(b) A licensee's failure to receive notification of renewal due to failure to notify the board of a change of address or name shall not
constitute an error on the part of the board or the bureau, nor shall it exonerate or otherwise excuse the licensee from renewing such license.
