848 IAC 1-1-10 Duplicate license

Sec. 10. (a) The licensee shall report, in writing, on the form supplied by the board, the loss of the original certificate of licensure or the
biennial renewal license.
(b) A license that has been lost or destroyed shall be replaced upon proper identification of the registrant.
