312 IAC 29-5-3 Modification to a Class II permit

Sec. 3. (a) The owner or operator of a Class II well shall submit an application to change a permit condition using a form prescribed by the division of reclamation before: (1) changing the injection zone; (2) adding an injection zone; (3) changing the maximum allowable injection pressure; or (4) changing the maximum allowable injection rate; of a permitted Class II well. (b) Notice of the request to modify the operation of a permitted Class II well under this section shall be provided according to the requirements of section 2 of this rule. A copy of the notice and proof of notification shall accompany the application to modify operation of the Class II permit.
