LAC 40:I.3715. Schedule of Maximum Allowances

A. The reimbursement allowances for home and vehicle modifications are determined by obtaining bids for the specific modifications. Maximum allowable reimbursement schedules cannot be preset or predetermined. A minimum of three bids should be obtained by the carrier/self-insured employer whenever possible. B. The payment agreed to by the carrier/self-insured employer represents the maximum allowance and constitutes payment in full for the services. No additional charge to the claimant is allowed. No charge shall be submitted to the carrier/self-insured employer before the service has been performed. All home and vehicle modifications require prior authorization.
