LAC 42:VII.315. Surrender of a License

A. A license may not be surrendered without the prior approval of the board. B. If a request to surrender a license is approved, the person is immediately eligible to apply for a license, unless the board or division has placed a condition that the applicant shall have to fulfill in order to reapply.
