LAC 46:V.4501. Complaints

A. All complaints made to the commission must be made on the complaint form provided by the commission. B. All complaints shall include documents which support the complainant’s claim. C. The commission shall mark each complaint received on the date it is received which shall designate the date upon which the complaint is filed. D. The commission shall assign a case number for each complaint received.
