A. Applications for Licensure. An applicant for licensure must submit a completed application that meets the requirements of R.S. 37:3556 along with payment of the fee required by R.S. 37:3562. The application must include: 1. proof satisfactory to the board that the applicant has satisfied the requirements for licensure; 2. an official transcript from the massage therapy school showing completion of the course of instruction. B. A license application shall be considered valid and pending for a period not to exceed 45 days. After 45 days, the license application will be considered expired and a new application will be required to proceed with the application process.
