LAC 46:XLV.2543. Renewal of License

A. Every license issued by the board under this Subchapter shall be renewed annually on or before the last day of the month in which the licensee was born by submitting to the board: 1. a renewal application in a format prescribed by the board; 2. the renewal fee prescribed in Chapter 1 of these rules; and 3. documentation of not less than ten contact hours of approved continuing professional education within the past twelve months as prescribed by Subchapter G of these rules. B. Renewal applications and instructions may be obtained from the board's web page or upon personal or written request to the board.
