LAC 46:XLV.6027. Required Information

A. Each physician shall report to the board annually, as a condition to the issuance or renewal of medical licensure, whether or not he or she is engaged in collaborative practice with a genetic counselor and, if so, such information as may be requested by the board. B. The information required by this Section shall be reported in a format prepared by the board, which shall be made part of or accompany each physician’s renewal application for licensure.
