LAC 46:XLV.7308. Required Reporting and Registration

A. Each physician or podiatric physician shall report to the board annually as a condition of the issuance or renewal of medical licensure, whether or not the physician performs or intends to perform office-based surgery. B. Each physician or podiatric physician who performs or intends to perform office-based surgery must provide the following information in a format designated by the board: 1. the medical specialty and types of procedures for which the physician has completed surgical training as required in this Chapter. Alternative credentialing for procedures outside the physician’s core competency must be separately applied for; 2. the location(s) where the physician performs or intends to perform office-based surgery; and 3. such other information as the board may request.
