LAC 48:IX.717. Closure of Requests for Supports and Services

A. Initial requests may be “closed” due to: 1. insufficient information based upon the person/family consistently not complying with obtaining needed information and/or participating in scheduled appointments; 2. denial for system entry (SOD) has been determined; or 3. request of the individual and/or family. B. If the person does not respond to the initial redetermination letter within 30 calendar days, at least two additional attempts to contact the individual will be made prior to case closure within the next 30 calendar days. The additional attempts to contact the individual will utilize more than one mode of contact.
