LAC 48:I.4283. Individualized Service Plan Review

A. The individualized service plan shall be reviewed and updated at least quarterly and whenever there is a change in problems, goals or approaches as indicated. B. This review shall be done by the person indicated on the plan as the individual primarily responsible for carrying out the plan. C. This review shall be accomplished by reviewing the individual reports of all persons responsible for meeting the needs of the participant. These reports shall include any reports from physicians, social service designees/social workers, nurses, therapists, dietitians, and family members as well as incident reports.
