LAC 48:I.4907. Application Procedure

A. The applicant shall submit a copy of a request for licensure to the Department of Health and Hospitals, Health Standards Section, P. O. Box 3767, Bin #27, Baton Rouge, Louisiana 70821-3767. The request shall include descriptions of: 1. the target populations to be served; 2. geographical areas (regions) to be served; 3. address(es) of the office site(s) to be used; 4. administrative file as described under §4943; 5. the provider's policies and procedures manual; 6. the requested program and services to be provided as outlined in §4953; 7. the provider's plan for staffing as outlined in §4959.
