LAC 48:I.5513. Disclosure of Ownership

A. Any facility which is funded, either directly or indirectly, by the Department of Health and Human Resources must supply to the Licensing and Certification Section full and complete information and promptly report any changes which would affect the current accuracy of such information, as to identify: 1. of each person having (directly or indirectly) an ownership interest of 10 percent or more; 2. in case a facility is organized as a corporation, of each officer and director of the corporation; 3. in case a facility is organized as a partnership, of each partner.
