LAC 48:I.8717. Changes in Licensee Information or Key Personnel

A. A license shall apply only to the location stated on the application and such license, once issued, shall not be transferable from one place to another or from one person to another. B. Any change in the facility regarding the following shall be reported in writing to the department no less than five days prior to the change: 1. entity name; 2. doing business as (dba) name; 3. geographic address; 4. mailing address and/or electronic address; and 5. telephone number(s) and fax number(s). C. For a change in facility address, a new license with a new license number will be issued upon receipt by the department of a new completed application and the licensing fee required for a change of address. The anniversary date shall be changed to reflect the date of issuance of the new license. D. Any change regarding the ABI facility’s key administrative personnel shall be reported in writing to the department within five days of the change. Key administrative personnel include the facility director and the nursing director. The facility’s notice to the department shall include the individual’s name, address, hire date and qualifications. E. Any request for a duplicate license shall be accompanied by the required fee.
