LAC 48:I.8735. Fees

A. Any remittance submitted to the department in payment of a required fee shall be in the form of a company or certified check or money order made payable to the “Louisiana Department of Health”. B. Fee amounts shall be determined by the department. C. Fees paid to the department are not refundable. D. A fee is required to be submitted with: 1. an initial application; 2. a renewal application; 3. a change of controlling ownership; 4. a change of name or physical address; and 5. each offsite residential location. E. Submission of fees that are returned for non-sufficient funds may result in the license being denied, either initially or at time for renewal, revoked or suspended.
