LAC 49:I.1311. Issuance of Certificate of RegistrationPermit [Formerly 49:8.0050]

A. A certificate of registration-permit shall be issued upon receipt of an application provided that no certificate of registration-permit be issued until inspection has been made of the tanning facility and it has been found to be operating in compliance with the provisions of the Act and these regulations. B. The certificate of registration-permit shall be displayed in an open public area of the tanning facility. C. An annual certificate of registration-permit shall be issued upon receipt of an application of forms provided by the department for this purpose and required renewal fees. D. A certificate of registration-permit shall be issued only to the person or persons responsible for the operations of the tanning facility and shall not be transferable.
