A. LEAs may implement the use of the standardized school health forms to eliminate the duplication of information submitted to schools and school nurses relative to health information and screenings, allergies, illnesses, sports physicals, medication administration, and prescribed procedures. B. These forms will be made available for download via the Internet on the LDE website and on the Louisiana Department of Health and Hospitals website.
