A. Effective for dates of service on or after December 1, 2011, the Medicaid Program shall provide reimbursement for fluoride varnish application services rendered by qualified health care professionals in a physician office setting. B. Reimbursement for fluoride varnish application services shall be a flat fee based on the appropriate HCPCS code.
