LAC 50:XI.10501. Provider Enrollment [Formerly §10301]

A. In order to enroll and participate in the Medicaid Program, an FQHC must submit a completed provider enrollment packet that includes a copy of the HRSA grant approving its FQHC status. B. The effective date of a FQHC’s enrollment to participate in the Medicaid Program shall not be prior to the date of receipt of the completed enrollment packet.
