LAC 50:XI.16501. Provider Enrollment [Formerly §16301]

A. In order to enroll and participate in the Medicaid Program, a RHC must submit a completed provider enrollment packet. B. The effective date of enrollment to participate in the Medicaid Program shall not be prior to the date of receipt of the completed enrollment packet.
