LAC 51:XXXI.305. Renewal of RegistrationPermit [formerly LAC 49:I.1313]

A. The registrant shall file applications for renewal of certificate of registration-permit on forms provided by the department. The application shall be sent to the mailing address of the principal registrant listed on the last application for registration-permit submitted. B. An annual renewal fee of $110 shall accompany each annual renewal. Make check or money order payable to the Food and Drug Unit/Department of Health and Hospitals. C. Provided that a registrant files an application with the department in proper form not less than thirty days prior to the expiration date stated on the certificate of registration-permit, the certificate shall not expire pending final action on the application by the department.
