Burnes, Commissioner of Insurance Date: August 14, 2007 Re: New Insurance Producer License Renewal Application Forms The Division of Insurance ("Division") has begun accepting the National Association of Insurance Commissioner's Uniform Application for Individual Producer License Renewal/Continuation and Uniform Application for Business Entity Insurance License Renewal/Continuation ("Uniform Renewal Applications") for the renewal of resident and non-resident individual and business entity insurance producer licenses. The Uniform Applications are available on the Division's website at www.mass.gov/doi.
