Operations Plan

(1) An applicant for a license under 700 CMR 19.03 shall submit an Operations Plan with their application. (2) MassDOT shall develop and publish on its website the form and minimum requirements to be included in the Operations Plan. (3) The Operations Plan shall be prepared by the quarry. (4) The Operations Plan (Quality System Manual) shall describe in sufficient detail the following minimum requirements in the following order: (a) Aggregate quality control procedures: and (b) Aggregate manufacturing in the mine and the processing plant, covering each of the following steps: 1. Extraction; 2. Processing; 3. Stockpiling; 4. Blending; 5. Sales / Delivery; 6. Plant Flow Diagram; and 7. Current Contact Information. (5) If there is a significant change to the operations plan during the license year, a new Operations Plan shall be prepared and submitted to MassDOT prior to implementing the change.
