The forms authorized in M.G.L. c. 106, § 9-521(a) and (b) and any amendments thereto and forms prescribed by the filing office shall be acceptable by the filing office. Other forms will not be accepted. Copies of these forms are available on the Office of the Secretary of the Commonwealth’s website at: www.sec.state.ma.us/cor.
