Change of Program Name

(A) A Licensed or Approved Provider may not change the name of any program licensed pursuant to 105 CMR 164.000 without receiving express prior approval by the Department. (B) An application submitted by a Licensed or Approved Provider for a change in name must be filed with the Department at least 90 calendar days in advance in a manner prescribed by the Department and shall comply with all applicable provisions of 105 CMR 164.000.
