Notification

A licensee shall notify the Commissioner orally within seven days of any change in its ownership, name or location and of any substantial change in its facilities or activities regarding care and treatment ofdogs and cats. If requested by the Commissioner after said notification, the licensee shallpromptly submit to the Commissioner a new or amended application, together with written documentation reflecting such change. This written documentation must be received no later than 30 days after a request by the Commissioner.
