Application Information Required

Any client seeking homemaker services shall be required to provide sufficient information to allow the Department to assess the applicant's need and eligibility for homemaker services. In addition, an individual applying for homemaker services may be required to provide such authorization(s) for collateral contacts as are necessaryfor the Department to verify or assess the individual's need for homemaker services.
