Transitional Living Program: Scope of Services

A transitional living provider must, at a minimum, provide the following services in accordance with each member’s needs as documented in the member’s service agreement: (A) transitional living services, as described in 130 CMR 422.431 through 422.441; and (B) transportation to health care facilities when public transportation is unavailable.
