The completed form should be sent to the Compliance & Enforcement Unit by e-mail at pcinform@mdinsurance.state.md.us Submission Deadlines are as follows: Data as of July 16, 2012 due July 31, 2012 Data as of October 16, 2012 due October 31, 2012 Data as of January 16, 2012 due January 31, 2013 If the company or association is unable to provide any or all the information, it must submit a detailed explanation as to why it is unable to provide this information and state what information it can provide. Supplied data is confidential commercial data protected under State Government Article, §10-617(d), Annotated Code of Maryland, except when aggregated with data from all other insurers in a manner that does not permit any individual company information to be identified.
