This revised transmittal form replaces the transmittal forms required by the MIA Bulletin 99-22. Samples of these forms as well as instructions for the completion of these forms are attached for your reference. Insurers and Rating Organizations are to commence using these forms for all rate and form filings that will be received by the Rate and Form Unit, on or after September 15, 2001. Any filing received on or after September 15, 2001, which has not been submitted with the completed revised transmittal form (4/2/01), will be considered incomplete and not filed with the Commissioner. These incomplete filings Will Not Be Reviewed until the correct filing transmittal forms have been received. If a filing is determined to be incomplete, the MIA will notify the filer what corrective action must be taken. Please communicate these new filing requirements to all company personnel who are responsible for submitting rate and form filings to the Property and Casualty Section of the MIA. This filing information will be available, along with the MIA’s Property and Casualty Compliance Guide, on the National Association of Insurance Commissioners’ website, www.NAIC.org, on or about August 13, 2001.
