R 333.5308 Utilization logs.

Rule 308. A registrant shall maintain for inspection by the department current logs, which contain all of the following information for each radiation machine: (a) The machine registration number. (b) The identity of the radiographer to whom the machine is assigned. (c) The locations where used and dates of use. (d) The signature or initials of the individual certifying each entry.
