Application for annual grant; form.

Sec. 13. An application for an annual grant authorized under this act must be made on a form prescribed and furnished by the department. The department may require the applicant to provide any information reasonably necessary to allow the department to make a determination required by this act. History: 1990, Act 345, Eff.
